If this is your first time for counseling or coaching and you want to make sure it will be a good fit, I am happy to schedule a 15-20 minute phone consultation before scheduling the appointment.

Upon deciding to make an appointment please go to Client Forms section. Be sure to go the forms for your particular needs.

Counseling Clients
Read the Counselor Disclosure Statement and Notice of Privacy Practices Notices.

Print out the Attestation and Consent to Treatment signature page of the Counselor Disclosure Statement, the Attestation signature page
of the Notice of Privacy Notice. You can copy the entire documents if you desire but I only require that you bring in the attestation pages (go green).

Print out the Client Billing Information and the Client Intake Form and complete.
Bring the signed Attestation pages and the Client Intake forms with you to your first session.

Inclement Weather

In the case of inclement weather, particularly on wintry ice or snow days, scheduled client sessions may be conducted by phone. This can also be arranged if you are homebound with a sick child or have other physical or time constraints.

Call me by the morning of the appointment to arrange a phone session that day. At the time of your appointment, I ask that you call me and take steps to insure whatever privacy you prefer for your session. I conduct sessions with some clients by telephone on a regular basis because they are some distance away. They report satisfaction and progress with this service.

Coaching Clients

Print out the Personal Information Sheet, complete, and bring with you to the first session. You can review the Coaching Agreement and then we can discuss it in session.

SAP Evaluation Clients

Print out the Request for SAP Services, complete and fax or email back to me. I will follow up with a phone call to set up an appointment to complete the evaluation.
