Insurance
My services may be covered in full or in part by your health insurance or employee assistance benefits. I am an in-network provider for many plans and can bill as an out-of-network provider if needed. You will need to contact your insurance company prior to your first appointment. You can download the Mental Health Benefits Worksheet to use when you speak with the customer service person for your insurance company. Bring it with you to the first appointment. Coverage quoted is not a guarantee of payment. Once you have determined your benefits and reimbursement/co-pay then call me so that we can finalize payment arrangements prior to your first session. I am happy to assist you in billing your insurance company however; you are financially responsible for all fees. Payment arrangements must be determined prior to the first appointment and be prepared to pay the determined fee or co-payment.

Payments
I accept payment for therapy or coaching at each session by cash or check. You may also submit a secure on line payment using your debit, credit card, or PayPal account prior to your session using PayPal. You do not have to have a PayPal account to use your debit or credit card.
